Madonna Ptak Morton Plant Rehabilitation Center is located on the campus of Morton Plant Hospital at the corner of S. Fort Harrison and Corbett Street. Following an acute hospitalization, you may need continued inpatient physical and medical rehabilitation services to achieve your maximum potential and discharge home. The Center offers a full range of physical, occupational, speech, and respiratory therapy seven days a week in combination with complex nursing services. Here, you will find a team of dedicated professionals committed to delivering personalized care tailored to your specific rehabilitation needs. Most therapy is provided on a one to one basis and family participation is encouraged. Whether recovering from orthopedic procedures including joint replacement and amputation, cardiac surgery, stroke or other neurological events, debility from other surgical or medical conditions or needing complex medical management and rehabilitation related to any number of disease processes including tracheostomy weaning, dialysis, or complex wound care, you will find the professionals at Madonna Ptak Morton Plant Rehabilitation Center ready to provide high-quality, compassionate care.

A part of the BayCare Health System, Madonna Ptak Morton Plant Rehabilitation Center is Medicare and Medicaid certified and accepts most insurances. Patients are admitted seven days a week and your physician may continue to follow your care at the Center or may choose to entrust your care to our Medical Director team of three physicians and a Nurse Practitioner who is onsite on a full-time basis. Our location and affiliations provides many specialist consultations onsite and quick access to emergency care if needed.

Nursing care is provided 24 hours a day, seven days a week by a highly skilled staff of RNs, LPNs, and CNAs. Specialty wound care is provided on a daily basis by two dedicated full-time wound nurses to assist the bedside nurse and a RN nursing supervisor is on duty at all times to support the nursing staff and provide any assistance you or your family may need.

A well-planned, coordinated discharge is key to a successful return home and our skilled nurse case managers and social worker will work with you and your family to identify, develop and implement your discharge plan.
